Anti Smooth Muscle Antibody (ASMA) is an autoantibody — an antibody that the immune system produces against the body's own tissues. This is the most clinically important distinction — because the treatment is completely opposite. Viral hepatitis is treated with antivirals; autoimmune hepatitis is treated with immunosuppressants (steroids). Giving steroids to a viral hepatitis patient makes things worse. Giving antivirals to an AIH patient does nothing.

Do not start steroid treatment based on ASMA alone — the diagnosis must be confirmed first.
